FOOTBALL & CHEER REGISTRATION

Online ​Registration Begins April 1st - June 30th,  If you do not register by the end date you may have to be put on a waiting list and participation is not guaranteed.

Football All Divisions:
$180.00 for the first participant, $150.00 for each additional participant. $420 max per family.

Cheer Participants:

$90 per participant  
 

Fees must be paid at equipment pick up date in July and a copy of participants birth certificate must also be submitted at equipment pick up (not online). No exceptions. 

The equipment pick up dates are  July 17th 10am-2pm 

All Freshman Lions 1st-2nd graders equipment pick up is 10am-11am. 

Varsity Wolverines 5th-6th Grade  11:30am 12:30pm.

JV Lions 3rd-4th graders 1pm-2pm.

Cheerleaders all grades 2:30pm-3:30pm.

All Cheer participants will be notified by email when fees are due.

FOOTBALL/CHEER EQUIPMENT LEASE AGREEMENT:  By accepting the terms here, I, the undersigned parent(s) or legal guardian(s), hereby guarantee the safekeeping and timely return or reimbursement of the replacement value of any and all equipment issued to my child by the Chesterland Lions Youth Football Program. I will not modify or mark the equipment in any manner to make it unusable or undesirable for future participants. Football participants -helmet with chin strap, shoulder pads, football pants with pads and practice jersey. For Cheer participants / uniform top, skirt and pom poms.  Equipment shall be returned as issued, excepting for normal wear and tear, clean and in good condition to Chesterland Lions Youth Football Program. Additionally I understand that legal actions may be taken against me to recover either the leased football equipment or the replacement cost of the retail value of said equipment and I will be responsible for Chesterland Lions Club fees to recover the funds.
 

RELEASE OF RESPONSIBILITY EMERGENCY MEDICAL AUTHORIZATION.
In the event of injury, as the parent or legal guardian of the above child, I authorize local professional emergency teams to tend to my child's medical needs, which may require transport to a medical facility. If reasonable attempts to contact the parents or legal guardian at the above listed phone numbers or email are NOT successful, I give consent for the administration of treatment deemed necessary by the treating facility, or health care provider.
 

Does You child have any allergies (penicillin, bee stings, etc)?
Does your child have Asthma or require the use of an inhaler?
Is your child Diabetic or require medication for diabetes?
Does your child have any other physical limitations or medical conditions?
 

If you can answer yes to any of these questions or have any other conditions please submit and explain to your head coach.

INTENT TO INFORM: I acknowledge that I am fully aware of the potential dangers of participation in football/cheer, as in any sport, and I fully understand that participation may result in serious injuries, paralysis, permanent disability and/or death.  Furthermore, I fully acknowledge and understand that protective equipment does not prevent all participant injuries, and therefore I do hereby waive, release, absolve, indemnify, and agree to hold harmless the Chesterland Lions Club, the  coaches, local, league, officials, and any and all organizers, sponsors, supervisors, participants, and persons transporting the above named participant to and from activities, from any claim arising out of any injury to my/our child whether the result of negligence or for any other cause.
 

I, the undersigned, hereby grant the Chesterland Lions Youth Football Program permission to use photographs of my child as elements of an assemblage/collage/photographic piece. I also give them permission to put the finished photographs to any legitimate uses they may deem proper. Further, I relinquish and give them all right, title and interest I might have in the finished pictures, negatives, reproductions, and copies of the original prints and negatives, and grant them right to give, sell, transfer, and exhibit the negatives, original prints, or copies and facsimiles thereof, to any responsible individual, business film, or publication, or to any of their assignee's.  
 

Lion’s Club reserves the right to place athletes on teams suitable for their age, weight and experience.
 

I understand that there will be no refunds if my child quits the program.​

If you have more than one participant you will have to register each participant.  The Chesterland Lions Club thanks you for all your support and we are striving for a safe and great football season.  After registering scroll to the top of the page for the confirmation and you will also receive an email confirmation shortly after.
